A. Churches are automatically tax exempt, and need not to file taxes nor report to the Internal Revenue Service. Churches must establish a separate 501(c)(3) called a Public Benefit 501(c)(3) in order to be eligible for 100% of grants.

ONE: The name of the corporation is PDI, Inc. The address for the initial registered office is 500 Arba Street, Montgomery, AL 36104 . The corporation is organized pursuant to the Alabama, Nonprofit Corporation Code.
TWO: This corporation is a nonprofit public benefit corporation and is not organized for the private gain of any person. The corporation is organized under the Nonprofit Public Benefit Corporation Law for charitable and educational purposes to aid the poor and disadvantaged individuals and families towards a life of self-sufficiency. PDI, Inc. is a 501(c)(3) tax-exempt organization. The purpose of this proposal is to request an application for proposal in order to apply for funding. These funds would be utilized to implement an after school care program. This program will provide after school care for children between the ages of 6 to 17 years old in Montgomery County.
This program will provide tutorial, mentoring, computer literacy and counseling services. The program is designed to reach all aspects of a young person’s life and reinforce positive messages in each one. It will also provide educational, cultural, social and athletic activities to reflect their developmental levels and personalities.
The program will operate throughout the year, becoming a summer program when school is out.

BRIEF PROGRAM DESCRIPTION
This program is designed to nurture the spirit, educate the mind, promote the development of the whole child, and to educate all children by stimulating the intellectual curiosity of each child from infants to age twelve (12). The program is designed to address the concerns of all families in our program. With the decrease of extended families, the increase in the rate of families moving, and more families experiencing stress, drug abuse and crimes; we have a concern that children in these families will experience high rates of delinquent behavior. Primary prevention and early intervention can help children increase coping skills and to emulate healthy behavioral patterns instead of negative ones. This program endeavors to lower the incidence of emotional disorders offering a balanced development program which stimulates each child’s intellectual curiosity and the desire for lifelong learning.
It also encourages self-esteem and confidence by recognizing the unique potential and worth of each child by reducing stress and promoting conditions that increase confidence and coping skills.
Finally, this program will provide developmentally appropriate programs that will stimulate each child’s intellectual curiosity and their desire for lifelong learning by providing various avenues of growth and development stimulation for infants, toddlers, preschoolers, and after schools, in a faith based environment.
PROJECT VALUES AND PRINCIPLES
We value the parents as the cornerstone of a successful and productive community. The proper growth and development of children within the family structure from prenatal to adolescence is a proven asset to any community. The parents of Faith Day Care, Inc will be culturally and economically diverse and take many forms. Our aim is to provide all children with quality child care service.

7. 9:15-9:45-Show and Tell Time. To develop language skills, children are encouraged to bring items from home and discuss with the other children in the group. This is also a time for children to discuss upcoming events or events that have happened in their lives.
8. 9:45-10:15-Children are introduced to colors, shapes and number as related to the learning environment. Children are placed into learning center during this time.
9. 10:15-10:30 AM – Clean-up time. Our staff are cleaning up the centers while the children are utilizing the restroom and preparing for outside play (if weather permits).
10. 10:30-11:15AM-Outside Play (If weather permits) or Free Play
11. 12:00-1:00PM. – Nutritional meals are served to the children daily during “Lunch” time
12. 1:00-2:30PM daily. Children will have rest time.
13. 2:30-2:45 PM - Informal snack
14. 2:45- 5:00 Assessment period for teachers
The eligibility criteria for participation in the Program are that individuals live in a radius of 30 miles. For those parents who receive free or subsidized child care, he/she would have to qualify by attending school or work to receive a scholarship. As long as the parent(s) is in school or working, the scholarship will continue until the child reached public school age and/or is expected in the public school system.
STAFF DESCRIPTION AND PROJECTED SALARIES
The Program Executive Director’s primary responsibility will be to oversee the program. The Program Executive Director will secure funding sources for various projects that will take place and will insure that the funds are spent appropriately. In addition, the Program Director will conduct program evaluations to insure that the project is conducted in accordance with the goals and objectives of the program.
The Administrative Assistant will be responsible for all front office duties. She will serve as a support for the Program Director and Teachers.
The day care providers/teachers (4) responsibilities will consist of but, not limited to providing various educational, enhancement, and enrichment programs or services. He/She will be responsible for developing and implementing programs that address early childhood education. He/She will maintain a file on each child /participant’s goals and their progress.
The Custodian will be responsible for the cleaning of the facility. The custodian will keep an inventory of all job related supplies needed and forward all request to the Administrative Assistant.
After school teachers, will provide children with assistance in completing homework assignments. They will be available to assist the children in areas of difficulties. After school teachers, will maintain a report on the progress of each participant. Projected Salary: $12,000 annually (2) Tutors
The program will cost $210,000 for the following items:

Equipment needed to maintain the program includes:
TV/VCR, file cabinets, typewriters, adding machines,
copier, desks, picture, and computers with laser printers.
Office/Program Supplies $6,500
Top quality supplies and materials need to be
purchased in order to provide children with the
quality child care.
Other equipment needed for the program includes:
Recreation equipment, signs, commercial carpet,
playground equipment, Library and resource supplies
and equipment, locks and lockers, water fountains,
kitchen and dining room equipment, and restroom
To provide all children with access to information that
enhances their learning experiences through photocopied
printed materials. In addition, this allows material to view
by parents and others involved in the children’s lives.
For external communication.

writing, and craft material thru magazines
This includes the cost of $30,000 for (2) vans
and the drivers salary of $5,000. In addition to
transportation being provided to and from the
center, participants will be taken on monthly
field trips to sites within a 50 mile radius of the center.
Conference and training $3,600
These expenses in include airfare fees ($1,100),
training seminars ($1,200) and statewide travel
at a rate of .21 per mile.
The teachers and directors will travel to meetings
and training seminars during the year. These
activities will add to the quality of the program
and will provide the innovative ideas for continued
education and the success of the program.
The annual cost to maintain proper coverage on
the facility and users.
